An England ICS, has built a new Orthopaedic Centre of Excellence (CoE) with 6 operating theatres & 50 beds and associated support rooms. The target official opening is in just 4 months time. New staff recruitment has only achieved 45% of target levels required for opening.The current orthopaedic elective surgery waiting list is now over 1 year, an increase of 10% in last 12 months. There is public and NHS pressure to open new Orthopaedic centre at full capacity. The CoE finances are already over budget due to high cost of construction. There is tremendous financial reluctance to temporarily in-source more staff as it will negatively impact other cost centres within the Trust.

Surge solution provides for: 47 nurses, 5 surgeons, 5 anaesthetists, 2 admin, 2 drivers, 6 physio & 4 therapy assistants. After 6-12 months when contract completed, and when CoE has completed full nursing recruitment, Surge exits and CoE continues as originally planned.

Limited theatre space and wards constantly challenged with critical surgeries and bed shortages. Post COVID, staff (surgeons & nurses) are exhausted and unwilling to work long or extra shifts.Difficulty recruiting additional or part time staff to the country. To have any measurable reduction in waiting list, in next 12-18 months, they need extra theatres with accompanying ward/recovery beds and extra staff. No capital funds available for building additions and equipping the theatres. Hospital performing higher % of complex surgeries given aging demographics of the local population. Average LOS per ortho patient is over 4 days with slightly higher than average UK reoperation rate. Lower than average number of ortho surgeries performed per day versus other UK hospitals is a factor of limited staffing (unable to cover illness, vacation, training coverage).Given demographics of region, current focus is on improving oncology treatment rate with little or no additional funds to support any initiatives to address orthopaedic waiting lists.
